Division I (5th & 6th Grade) – May not turn 13 before July 1, 2025 (ineligible to cheer if born before July 1, 2012)

Division II (3rd & 4th Grade) – May not turn 11 before July 1, 2025 (must cheer for Division I if born before July 1, 2014)

Division III (Kindergarten - 2nd Grade) – May not turn 9 before July 1, 2025 (must cheer for Division II if born before July 1, 2016) and must be enrolled in Kindergarten for the 2025 school year.

Behavior Agreement

I agree to the Code of Contact for BMF & the WPYFL. I agree that my behavior and the behavior of my guests during practices, games and events associated to BMF & the WPYFL will directly affect my child's ability to participate in future events. I fully understand that removal of any of these activities may result in the expulsion of my child from the team. I WILL act in a manor that is respectful and reflects accordingly to BMF & the WPYFL. I will not use obscene language, yell excessively, participate in any altercations or cause a scene during games. I fully understand that this a YOUTH sporting event and my actions will reflect as such.

Remind App Agreement

I agree to join the BMF Group in the Remind App. I understand this is a FREE service and can join by simply text messaging @bmf2025 to the number 81010. I also understand that Remind participation is a requirement for the 2025 season.

Fundraiser Participation

FUNDRAISERS / BUYOUTS ARE MANDATORY.
Please select either "Fundraisers" or "Season Buyout".
Fundraisers not turned in by deadline WILL result in loss of game time or removal from the team. Any fundraisers not turned in will be considered theft and will be handled in such a manor with local authorities / courts.

2025 BMF / WPYFL CHEERLEADER & PARENT AGREEMENT

I/We, understand that fighting, arguing, unruly behavior, obscene language and all behavior deemed to be detrimental to the growth of BMF / WPYFL at any practice, game or event, may result in expulsion from future activities up to and including ability to play for BMF / WPYFL and I/We will not receive any type of refund.

I/We, certify that I/we, am/are the legal parent(s) or guardian(s) of the above named participant and all information stated above is correct.

I/We will adhere to all BMF / WPYFL bylaws, rules, regulations and rulings.

I/We hereby confirm that I/We have read, understand and hereby agree to abide by the BMF / WPYFL code of conduct.

I/we are also aware that my/our failure to abide by the BMF / WPYFL code of conduct may results in penalties as outlined in the bylaws up to and including my/our dismissal and participation in the BMF / WPYFL in any capacity.

I/We acknowledge that the BMF / WPYFL has the right to impose punishment, up to expulsion of my son or daughter from any activity that is overseen by the BMF / WPYFL.

I/We also acknowledge that my/our son or daughter is attending school at the school which has been submitted on this form.

I/We understand that if my/our son or daughter moves to a different school district during the 2025 season the organization where the child participates must notify the BMF / WPYFL within 5 days of enrolling at the other school district.

I/We further understand if the BMF / WPYFL is not notified within 5 days that my/our son or daughter may be suspended for the remainder of the season.

2025 BMF / WPYFL CHEERLEADER LIABILITY WAIVER

I/We, the parents/guardians of the participant, understand that the game and sport of football is an inherently dangerous activity and that there are genuine and real serious risks to anyone who engages in this activity. We also understand that football is the highest risk sport for injury at the high school level. The game of football calls for heightened physical contact and collisions, hence, we acknowledge and understand that the risk of serious physical injury, including catastrophic injury resulting in permanent paralysis, brain injury, or death, does exist.

I/We fully acknowledge and understand that protective equipment does not prevent all participant injuries I/We knowingly assume responsibility for any and all such risks and any and all resulting injuries, including death. I/We promise to accept and assume responsibility and risk for injury, death, illness, disease, damage to property arising from my traveling to, participation in, or returning from this activity. And I/We do hereby voluntarily choose to participate in this event in spite of the risks. Furthermore, we attest that the participant is physically fit and has sufficiently trained for this event. My child (the participant) does not have any medical record or history that could be aggravated by his participation in this activity.

I/We hereby grant my permission for any and all emergency medical/dental treatment and/or first aid to be administered to my child/participant, including authorizing any medical treatment facility/hospital to administer emergency treatment, for any illness/injury/accident resulting from participation in any BMF / Western Pennsylvania Youth Football activities.

All risks are hereby understood to my child and me. I/We do knowingly and willingly waive, indemnify, release, absolve and agree to hold harmless the BMF / Western Pennsylvania Youth Football League, and any and all organizers, coaches, volunteers, sponsors, property owners, property maintenance crews, players and participants for any claim arising out of injury, negligence or dissatisfaction while participating in and/or traveling to any BMF / Western Pennsylvania Youth Football League activity.
